How to check eligibility for Ayushman Bharat Card
1. First, go to the PMJAY website: https://pmjay.gov.in/.
2. Click on ‘Am I Eligible’ option.
3. On the new page, enter your mobile number.
4. Type the captcha code and click on ‘Generate OTP’ to get a code.
5. Enter the OTP.
6. Choose your state and district.
7. Look for your name, ration card number, and mobile number.
8. Check if your family is eligible for this scheme based on the search result.
9. You can also call the Ayushman Bharat Yojana call center at 14555 or 1800-111-565.
How to download Ayushman Bharat Card
1. First, download the ‘PMAJAY-Ayushman Bharat’ app from Google PlayStore on your phone.
2. Open the app and agree to the rules. Then press ‘LOGIN’ to move forward.
3. A login page will appear. Choose ‘Beneficiary’ and enter your mobile number and state. Click Next.
4. You’ll get an OTP on your phone. Enter it and click ‘NEXT’.
5. Create and confirm a lock code.
6. Finally, click on ‘Card Download,’ and your Ayushman Bharat Card will show up on the screen.
